Key Request

To better serve you, please read the following information before submitting a key request.

NOTE: No billable services are accepted on the Key Request Form. If your request requires a fee, you must use a Project Request Form.

How to request a key

  1. A Key Request Form should be filled out. The form is an interactive PDF which allows you to complete the required fields online, download and then print the form.

  2. The form must be signed by the Department Head and by the Building Coordinator.

  3. Each request must include a unique identifier known as the "key request number (No.)". The Key Request No. includes the date, building code or name, department room location and sequential numbers if sending more than one request per day. An example key request number: 04-03-23-BH-210-1 is created as follows - date (04-03-23), building code (BH), room location (210), sequential numbers starting with one (1), separating each number with a dash (-).

  4. Fax or email completed signed form to 706-721-2139 (for Health Sciences buildings) or 706-731-7970 (for Summerville/Forest Hills buildings) or keyandlock@augusta.edu.

  5. The Key Requestor will be notified via email when key is ready for pick up. The key will ONLY be available for pick-up for thirty (30) days after the date of the email notification.

Important Information

  • Keys to university buildings are the property of Augusta University and are issued only to employees. Keys are obtained at the JagCard Services Key Control and must be returned upon termination of employment or transfer to another department.

  • Do not duplicate or transfer keys.

  • To replace a broken or bent key, notify the Augusta University Lock Shop at 706-721-3638, or email keyandlock@augusta.edu to schedule a time to get your replacement key. No fees will be charged for broken or bent key replacement.

  • Unwanted, obsolete, or keys from transferring or terminating employees MUST be returned to JagCard Services Key Control. You will be given a receipt to end your responsibility for them.

Lost or Stolen Keys

Lost or stolen keys must be reported to JagCard Services Key Control. All fees due for lost or missing keys must be paid-in-full before any new or replacement keys will be issued.
