All Cadets entering the Army ROTC program must complete these forms prior to participating
in ROTC classes and activities. Be sure to download the forms to your computer and
open them in a PDF reader BEFORE typing in your information. These forms must be TYPED
(except signatures), printed, and then signed by hand. No digital signatures.
Once we receive the 139-R Form (below), we can use that information to submit you to begin the medical qualification process with the Department of Defense Medical Examination Review Board (DODMERB). This process consists of a physical, hearing, and eye exams conducted by approved medical providers in your area. These exams are done at no cost to the Cadet.
For incoming Cadets who are not yet DODMERB qualified, you will need to have your physician complete the DA Form 3425 Medical Fitness Statement (instructions) before participating in physical activities with ROTC. Please note that if our office has approved you to begin the DODMERB medical process, you can also take this form to your DODMERB physical to be completed.
